For some reason the item was thrown in a pile and forgotten. The Seller was able to locate the item and get it shipped. (Only thing that was not good was USPS - they moved it around for over two weeks - moving the package to different distribution centers (hit the Cleveland distribution center 3 times before eventually being delivered to me - kind of frustrating watching it being bouncing around Ohio)
